  • Vancomycin per pharmacist protocol management
  • Direct thrombin inhibitor per pharmacist protocol management
  • IV to PO programs
  • Therapeutic interchange programs
  • 24/7 clinical specialist back-up


  • Residency research or quality improvement project
  • Drug monographs
  • Medication use evaluations

Readmission Risk Assessment (RRA) Program

•    Multidisciplinary, organizational initiative to prevent medication-related readmissions
•    Involves targeted interventions by decentralized clinical pharmacists, specialists, and residents to improve the care of patients at high risk of readmission 

Teaching Opportunities

  • Continuing education conference
  • Resident case conference
  • Resident Teaching Seminar provided through MCPHS University
  • Precepting students


  • Residents are evaluated based upon standards set by ASHP for each residency program
  • Residents evaluate the preceptor and rotation at the end of each rotation
  • Longitudinal feedback is provided for on-call programs, staffing, teaching & presentations and summatively following each quarterly meeting
