Birth Certificates

After birth, you’ll fill out the Massachusetts Parent Worksheet for a Certificate of Live Birth—this is used to create your baby’s birth certificate, so it helps to write clearly and double-check names and details.
After the hospital files the birth record, a certified copy is usually available to order later.
