  • Treatment of minor illnesses and injuries  
  • Connections to services such as food, housing, education, etc.
  • Coordination of specialized patient care  
  • Forensic medical evaluations  
  • Medical or cognitive waiver evaluations for the US citizenship exam
  • Psychiatric and case management support

Anyone getting care through our program also has access to our case management services. Our case management team takes a holistic, patient-centered approach to health by addressing critical social needs and works to connect you to essential resources, such as food, housing, legal support, and more.
