How to Refer to WRAP

Internal Referrals (from within BMC Health System)

Providers should submit an Ambulatory Referral to Psychiatry: Psychosis Clinic.

  • From an outpatient context, referrals can be placed using “Orders.”
  • From an inpatient or emergency department context, place an order in the Disposition tab or in the Discharge Orders pane.

External Referrals

  • Make sure the patient gets registered at BMC:
    • If the patient will be new to BMC Health System, call 617.414.6060 to register them.
    • Upon registration, all new patients should make an initial appointment with a primary care provider.
  • Download our referral form (PDF).
  • Send the completed referral form and clinical information (relevant medical records) to the WRAP Program Manager. Referrals are not complete without clinical information.
    • Use the subject line “WRAP Referral Form from [name of your institution/practice]”.
    • If the patient has been hospitalized within the past month, we will need the most recent discharge summary.
    • You can send via fax to 781.559.0894 or by secure email to WRAP@bmc.org.

Once the referral has been reviewed and approved, WRAP clinic staff will call the patient directly to schedule in the WRAP clinic.
