Neurodivergent Urban Youth (NUY) Clinic

Provider Resources

Thank you for collaborating with the Neurodivergent Urban Youth (NUY) Clinic at Boston Medical Center in providing care to your patients. Review the following information before referring to the NUY clinic, in order to improve patient care and timeliness of the evaluation process and/or follow-up care.

In the NUY clinic we commonly evaluate patient’s ages 14-24 years old for:

  • Autism spectrum disorder (ASD)  
  • Attention-deficit/hyperactivity disorder (ADHD)  
  • Intellectual disability (ID)  
  • Learning disability (LD)  
  • A neurological, developmental, or psychiatric concern if the patient has an existing diagnosis of ASD, ADHD, ID, and/or LD  

How to Refer to the NUY Clinic

External referring clinicians: Use our external intake form and fax it to 617.638.6756 with ALL required paperwork 

Internal referring clinicians and EPIC users: Use EPIC referral and fax ALL required documentation that is not visible in the medical chart to 617.414.3661 

Required Paperwork

  • Encounter note that prompted referral
  • Current hearing and vision results (in office screening or consult reports)
  • Current IEP (including transition planning form) or 504 accommodation plan (most recent if graduated school)
  • Results of validated ADHD screening tool e.g., Vanderbilt (applicable for ADHD concern)
  • Original diagnostic report if seeking follow-up care

Follow-Up Care

If referring a patient to the NUY clinic for an existing diagnosis, please provide a copy of the original diagnostic report and reason for follow-up care in the referral.

Please Note

When scheduling a patient, we will make three attempts at contact to book an appointment. If we do not reach the patient/family, they will be removed from the NUY clinic wait list and the referral will be closed.
