Provider Resources

Pediatric specialty clinics typically require a provider referral. Pertinent clinical notes and referral information must be faxed to the clinic at 617.414.4502. Ensure the patient's home telephone number is clearly included on the faxed paperwork.  

Once the clinic receives the documentation, staff will contact the family to register the patient and schedule the appointment.
