Providers Outside BMCHS
If you do not practice at BMC or a CHC on OCHIN, please use ChartLink or fax referrals to BMC at 617.638.6756 and include the following information:

  • Your name, practice location, and contact information
  • The patient’s full name, DOB, contact information, and insurance (if known)
  • The clinic you’re referring to
  • Reason for referral

Boston HealthNet CHC Providers
If your community health center is on BMC’s Epic or OCHIN, you can submit a request directly through Epic. Just search for the department or program you’d like to refer to and use the button to submit. If your community health center is not on BMC’s Epic or OCHIN, please follow the instructions above.

Integrated Specialty Care for Your Patients

If your community health center is on BMC’s Epic or OCHIN, you can submit a request directly through Epic. Just search for the department or program you’d like to refer to and use the button to submit. Providers at these community health centers can submit through Epic:

  • Whittier
  • South Boston
  • Mattapan
  • Upham’s Corner
  • Roslindale
  • BHCHP
  • Codman
  • Dot House

If your community health center is not in the above list, please use ChartLink or fax referrals to BMC at 617.638.6756. Make sure to include:

  • Your name, practice location, and contact information
