Resources for Patients

For your baby’s first appointment, please bring:

• The name of your early intervention coordinator, if you have one
• A baby bottle and the formula brand that you use, if you’re bottle feeding
• Any questions that you have

Other family members are welcome to come to the first appointment. If you need another appointment, we'll schedule it before you leave. We'll also send a report to your baby's regular pediatrician, who you will continue seeing.
