Refer a Patient

To refer a patient to the Preventive Food Pantry, please follow these instructions:

From Logician

1. Open patient file
2. Click “Print”
3. The “Print” dialogue box will open
4. Click the + sign next to “Letters”
5. Click the folder next to “BMC”
6. “Print Items” column to the right will populate with a list
7. Scroll down to “Nutrition Resource Center Referral”
8. Click “Print”

The letter will say “Boston Medical Center, Nutrition Resource Center, Referral Form” at the top. Please give your patient this referral form to take with them to the Food Pantry.
