This, along with the ongoing Yawkey building entrance closure, will help us bring you an even better campus experience that matches the exceptional care you've come to expect.   • Current insurance cards
  • Current home address and phone number(s)
  • List of medications you take, and their dosage (how much)
  • Test results related to your current foot condition
  • Custom braces, arch supports, and shoes you are currently using
