The Geriatric Section's Clinical Program is known as Boston University Geriatric Services at Boston Medical Center. As geriatrics specialists, we understand your needs and your concerns as an older adult, and we will help you preserve the greatest degree of independence possible.
Integrated Model of Care and Case Management
Because the health needs of older adults are complex, we offer a comprehensive, coordinated team approach to care. Our doctors, nurses, case managers, nurse practitioners and social workers have geriatrics skills and experience to meet your primary care needs. We manage and coordinate the care you need in a variety of settings – in our ambulatory practice, your home, selected nursing homes or while you are a patient in the hospital.
As a patient in our practice you remain in our care even if you need to transition from one setting to another. To ensure your comprehensive care, we collaborate with many other health care providers such as private nurses, physical and occupational therapists, local visiting nurse associations as well as other community service agencies. If an emergency arises when our offices are closed, one of our doctors is always available by phone. As part of Boston Medical Center, we can provide you with easy referrals to our BMC colleagues in 70 medical specialties. If you require hospitalization, we will provide and/or coordinate your inpatient care, as well.
In short, our goal is to provide you with a feeling of security about all of your health care needs.
