Anita Bien-Aime, Pediatric Care Coordinator (BMC)
At BAP, Anita works diligently to meet the families to help them with their overall physical, mental and emotional health. She is passionate about youth, young adults and families accessing the services beneficial to the betterment of their lives and communities. Anita has experience in working with DDS, domestic violence agencies, families involved with DCF, and pharmacy and has her Master’s in Social Work from Simmons College.

Branden Washington, Pediatric Care Coordinator (BMC)
Branden has been in the healthcare field for over ten years with a wide range of experience providing community support services across all ages, having held positions in dialysis, Adult Family Care, home health care, and Pediatric Services of America. In his current role, Branden helps patients who have LTSS needs find services and resources with a concentration on patients who need to transition from pediatric to adult care.

Rene Martinez, Adult Care Coordinator (CBES)
René has worked in the human services field for over 36 years in numerous capacities, including lead Case Manager at multiple organizations. He currently works with patients with LTSS needs at Central Boston Elder Services and is bilingual in both English and Spanish.

Becca Desrocher, Adult Care Coordinator (Boston Senior Home Care)
Becca is a care coordinator for BSHC who assists members with their LTSS needs. She recently graduated from Salem State University where she received her Bachelor’s degree in Psychology. Prior to working for BSHC, she worked with a professor at SSU to help run a research lab where she studied animal and human behavior. Becca hopes to apply to graduate programs and further her education in mental health in the near future.

Sureidy Fernandes, Adult Care Coordinator (Ethos)
Sureidy works with adults with LTSS needs at Ethos, one of Boston’s Aging Service Access Points. She began her career working as an Outreach Community Coordinator and has previous experience working with the elderly. In addition to Human Services trainings, she has conducted home visits with older adults and worked with people with disabilities. Sureidy is bilingual in both English and Spanish.

Ambar Guerrero, Adult Care Coordinator (Boston Senior Home Care)
Ambar is responsible for outreach and case management for referred patients in regards to their health and wellness at BSHC. Before working with BAP, Ambar worked at Tufts Medical Center as a phone representative answering any and all questions consumers had. Ambar will be graduating this summer with her Associate’s degree in Liberal Arts.

Mirela Llupi, Administrative Coordinator
Mirela came to BAP from the Eye Clinic at Boston Medical Center, where she was a Patient Service Representative. Her previous experience includes administrative coordination and international relations for the Diplomatic Corps in Kosovo. Mirela has two Bachelor’s degrees (Modern Languages from Cambridge College and Economics from Boston University) and is fluent in Albanian, English, French, and Italian.

Winifer Polanco, Adult Care Coordinator (Central Boston Elder Services)
Winifer has worked in the medical field for over 5 years and has learned all about the Human Services field. She graduated with an Associate’s degree in the Human Services field and has acquired experience through different internships with LCSWs. Winifer works with members with LTSS needs at CBES and is bilingual in both English and Spanish.

Tiffany Gavin, Director (BMC)
Tiffany comes to the program with over 15 years of experience in healthcare ranging from corporate planning to operations leadership. Most recently, she directed the MACC 4C program, a care coordination program for medically complex children. She received her MBA from Duke University with a concentration in Health Sector Management and Operations.

Melanie Cannon, Program Manager (BMC)
Melanie is responsible for daily operations of the program and serves as the clinical point of contact for medical providers and health systems. Prior to Boston Allied Partners, Melanie worked on Strategic Grant Initiatives at Boston Medical Center with foundations, the state of Massachusetts, and the federal government.
