Anita Bien-Aime, Pediatric Care Coordinator (BMC)
At BAP, Anita works diligently to meet the families to help them with their overall physical, mental and emotional health. She is passionate about youth, young adults and families accessing the services beneficial to the betterment of their lives and communities. Anita has experience in working with DDS, domestic violence agencies, families involved with DCF, and pharmacy and has her Master’s in Social Work from Simmons College.

Branden Washington, Pediatric Care Coordinator (BMC)
Branden has been in the healthcare field for over ten years with a wide range of experience providing community support services across all ages, having held positions in dialysis, Adult Family Care, home health care, and Pediatric Services of America. In his current role, Branden helps patients who have LTSS needs find services and resources with a concentration on patients who need to transition from pediatric to adult care.

Brooke Sabia, Adult Care Coordinator (BSHC)
Brooke is responsible for outreach and case management for referred patients in regards to their health and wellness, all of whom are adults. Prior to this she was a female services counselor at MCI-Framingham, where she acquired knowledge in case management and group facilitation of the incarcerated population, specific to female offenders.

Sean Mahoney-Pierce, Adult Care Coordinator (Ethos)
Sean works with adults with LTSS needs at Ethos, one of Boston’s Aging Service Access Points. He began his career working in the homeless services and stabilization field and has experience working at an Independent Living Center, Boston Housing Authority complex, Action for Boston Community Development, and Boston Public Health Commission's Bureau of Substance Use Services.

Rene Martinez, Adult Care Coordinator (CBES)
René has worked in the human services field for over 36 years in numerous capacities, including lead Case Manager at multiple organizations. He currently works with patients with LTSS needs at Central Boston Elder Services and is bilingual in both English and Spanish.

William Ruhm, Adult Care Coordinator (BSHC)
William completed his Masters in Social Work at Boston University School of Social Work with a specialization in gerontological social work, where in addition to coursework he completed two internships conducting home visits with older adults and people with disabilities. William has an interest in integrating Narrative and Solution Focused therapy techniques into his work with older adults as a way to help these clients increase their confidence and self-efficacy.

Winsome Waldron, Adult Care Coordinator (Ethos)
Winsome has held various care management positions throughout the city of Boston and has vast experience serving older adults. Currently she is based at Ethos, one of Boston’s ASAPs located in Jamaica Plain.

Tiffany Gavin, Director (BMC)
Tiffany comes to the program with over 15 years of experience in healthcare ranging from corporate planning to operations leadership. Most recently, she directed the MACC 4C program, a care coordination program for medically complex children. She received her MBA from Duke University with a concentration in Health Sector Management and Operations.

Melanie Cannon, Program Manager (BMC)
Melanie is responsible for daily operations of the program and serves as the clinical point of contact for medical providers and health systems. Prior to Boston Allied Partners, Melanie worked on Strategic Grant Initiatives at Boston Medical Center with foundations, the state of Massachusetts, and the federal government.

Britani Wallace, Program Assistant (BMC)
Britani works with the Program Manager and Director on daily program operations and staffing training. Currently, Britani is working towards her Master’s Degree in Public Health at Boston University, studying Epidemiology and Biostatistics, as well as Mental Health and Substance Use. Britani’s previous experience includes working at a local pre-school/elementary school and volunteering at a local hospital in Michigan.
