The Foster Care Clinic provides timely, high-quality medical evaluation, transitional behavioral health support, and care coordination for Boston Medical Center patients entering custody with the Department of Children and Families (DCF). We work to support the health of children and adolescents during a transition to foster care or a new placement and works to honor each child's unique life experiences.

## What to Expect

We provide assessment and support around medical, behavioral, emotional, and developmental needs of children and adolescents, and assistance in navigating the next steps. Our focus is to aid in transitions to a new living situation while promoting healthy and safe connections to parents and caregivers.

At each initial visit, often referred to as a DCF 7-Day, patients and their caregivers will have the opportunity to meet with a medical provider, social worker and community wellness advocate. These visits include:

- Medical evaluation
- Immunizations and labs
- Screening for behavioral health and developmental symptoms
- Assessment and support around adjustment to out of home placement
- Brief behavioral health interventions to support transitions and cope with trauma
- Referrals to needed specialty care, including dental, vision, and behavioral health

Following the visit, our multidisciplinary team will coordinate with DCF, foster parents and residential programs to assist with accessing medical and behavioral health services. We work to support DCF and caregivers understanding of a child's physical and mental health needs, and work to link care back to the child’s medical home within BMC Pediatric Primary Care and/or Adolescent Clinic. Typically, DCF 30-days are held with the child's primary care provider who will continue to provide the child's ongoing medical care.
