 As children with sickle cell disease grow into teens and young adults, they will need to transition from pediatric care into adult care, which means finding the right care team to support them and address their health needs. BMC’s Transitional Care Clinic team, which includes both pediatric and adult sickle cell specialists, works closely with patients and their families during this time to help our young adult patients grow into mature, responsible adults who can manage their health on their own, while providing a safety net as they learn these new skills.
