# Treatments and Services

 Explore Pathways to Adult Care Clinic

 Our innovative program helps our patients adjust from pediatric care to care in the adult clinic by offering a seamless care team with both adult and pediatric providers. By providing care from both sets of providers in the “new” adult clinic, we aim to empower our patients and build trust in the new environment and new providers.

All the care we provide is tailored to each patient. We look at whether a patient is ready to make that transition to the adult clinic and help them make necessary changes for the level of responsibility associated with adult care.

Our multidisciplinary team includes physicians, nurses, nurse practitioners, case managers, with social service and pharmacy support and offers the following services in a “one-stop shopping” model of care:

- Antiviral therapies
- Primary care for adolescents with HIV or AIDS
- Connections to reproductive health care
- Connections to mental health services
- Pharmacy support
- Social services and case management
