 Explore Nursing Home ProgramOur interdisciplinary nursing home team provides care for medically complex patients who require subacute rehabilitation after a hospitalization or long-term care in a skilled nursing facility. Our patients benefit from continuity of care by remaining in our Geriatrics practice and within the BMC health system network.
