  Boston Medical Center’s [Diabetes Education](/node/177166 "Diabetes Education | Boston Medical Center") program aims to help you enhance your diabetes-related knowledge and skills, so that you can take good care of yourself, improve your general health, and reduce your risk of complications from diabetes.

This page provides instructions for sharing your blood sugar readings and other diabetes-related data with our clinic. If you are having difficulty uploading data from your device, please call the tech support phone number associated with your specific device.

 

 

 

 ## Continuous Glucose Monitors

 

 ### Dexcom

Giving our clinic access to your data can make your visits more efficient. Our clinic will send you a unique share code so that we have continuous, secure access to your data. If you have not yet received an invitation from us, you can send a secure MyChart message to your endocrinologist or diabetes educator and ask them to send the code. You can stop sharing with our clinic at any time.

##### Share Your Data Using a Mobile App

1. Download the free Dexcom CLARITY app onto your mobile device:

5. Select your date of birth and tap **Continue**.
6. Check the box “I consent to share my data with the clinic” and tap **Yes, Share My Data**.

##### Share Your Data Using a Dexcom Receiver and Your Computer

1. Create an account and log in on the [Dexcom Clarity website](https://clarity.dexcom.com/ "Dexcom Clarity (clarity.dexcom.com)").
2. Choose **Dexcom CLARITY for Home Users**.
3. Follow the instructions to download and install Dexcom Uploader software onto your computer.
4. Connect your receiver to your computer and the upload will begin automatically.

  ### Abbott Freestyle Libre

Glucose data can be shared with our clinic from your **FreeStyle LibreLink** and **FreeStyle Libre 2** apps by entering our clinic practice ID (**08883019**) into the FreeStyle Libre app on your phone. Providing us access to your diabetes data can make your visits more efficient. You can stop sharing with our clinic at any time.

##### Share Your Data Using a Mobile App

1. Depending on which device you have, download the appropriate FreeStyle LibreLink app onto your mobile device:

4. Select **Connect to a Practice**.
5. Enter our clinic’s practice ID: **08883019**
6. Confirm that you’re now sharing your data with “BMC Diabetes.”
7. Choose **Connect**.

##### Share Your Data Using a Receiver and Your Computer

1. If you are going to use a USB cable to upload data to your computer, you will need to install the LibreView Device Drivers on your computer. The Device Drivers locate the compatible device plugged into the computer and retrieve the data from the device to generate the reports. 2. Create an account at [LibreView](https://www.libreview.com/chooseCountryLanguage "LibreView (libreview.com)"):
    - Choose your country and preferred language, and then **Submit**.
    - Choose **Sign Up**.
3. Once you have registered and logged in, select **LibreView Patient Account**.
4. Choose **Menu** &gt; **Account Settings** &gt; **My Practices**.
5. Enter our clinic’s practice ID: **08883019**
6. Click **Add**.
7. Confirm that you’re now sharing your data with “BMC Diabetes.”
8. Choose **My Devices**.
9. Choose **Upload Device**.
10. Connect your receiver to your computer using the charging cable.
11. Select **Press to Begin Upload**.

 ## Insulin Pumps

 

 ### Medtronic MiniMed

The MiniMed Mobile App is only compatible with the following Medtronic insulin pumps:

- MiniMed™ 770G
- MiniMed™ 780G

*In order to share your data with our clinic, we need your CareLink™ username and password. We will use this information to add you to our clinic’s CareLink™ portal. Please provide your CareLink™ username and password to your healthcare provider or diabetes educator.*

##### Share Your Data Using the Mobile App

1. Download the free Medtronic MiniMed™ Mobile US app onto your mobile device:

2. Once installed, open the app. You will be asked to log in to CareLink™.
    - Select your country.
    - If you have a CareLink™ account, enter your account username and password and select **Log In**.
    - If you do not have a CareLink™ account, choose **Create an Account**. Follow the instructions to register a new account, then log in using your new account username and password.
3. Set up the MiniMed™ Mobile app.
    - Read the End User License Agreement (EULA) when it appears, then choose **Agree**.
    - You will see a screen showing what you need to set up the app. Choose **Get Started** at the bottom of this screen and follow the prompts.
4. Pair the insulin pump with your mobile device.
    - Select the MiniMed™ pump model that you are using.
    - Follow the instructions to put your pump into “pairing mode.” (On some mobile devices, you will receive a Bluetooth Pairing Request. Choose **Pair** to continue.)
    - Choose **Next**.
    - You will see a warning that dosing decisions should not be made based on this app. Read the warning and choose **OK**.
    - Choose **Agree** to confirm that you have read and understood the limitations of the app.

##### Share Your Data Using a USB Connection

Use this option if you are unable to use the mobile app. You will need to use the USB CONTOUR glucometer that was included with your pump.

1. To upload your pump data to [CareLink™ Personal](https://carelink.minimed.com/app/login "CareLink Personal: Data Management for People Living with Diabetes | MiniMed (carelink.minimed.com)"), you will need to create an account.
2. After logging in, follow the instructions to install the CareLink Uploader app onto your computer.
3. Follow the Uploader instructions to connect your pump to your computer and send your data.

  ### Tandem T:Slim (previously T:Connect)

In order to share your data with our clinic, we need your T:Slim username and password. We will use this information to add you to our clinic’s T:Slim portal. Please provide your T:Slim username and password to your healthcare provider or diabetes educator.

##### Share Your Data Using the Mobile App

1. Download the free Tandem t:slim app onto your mobile device:

2. Unlock your insulin pump and navigate the menu: **Options** &gt; **Device Settings** &gt; **Bluetooth Settings**.
3. Turn on **Mobile Connection** (enable Bluetooth).
4. Log in to the t:slim app using your web application username and password (or register a new account from within the app).
5. Follow the prompts to pair your t:slim X2 insulin pump with your mobile device.  
      
    **Note:** Only one device at a time can be paired with the t:slim app.

  ### Insulet Omnipod (via Glooko)

The ProConnect code for our practice is **bumc**. You will need to enter this code whether you are sharing your Omnipod data via the Glooko smartphone/tablet app or using a computer.

##### Share Your Data Using Your Computer

1. Visit the [Glooko Login Page](https://my.glooko.com/users/sign_in "Log In | Glooko: Makes Diabetes Management Easier (my.glooko.com)"). (You can also use this page to register a new account.)
2. Log in with your username (email address) and password.
3. Click on the triangle and choose **Settings**. (The letters inside the circle will be your initials.)
4. Under **Account &gt; Primary Provider**, enter the ProConnect code for our clinic (**bumc**).
5. Scroll down and click **Save**.
6. Under **Apps &amp; Devices**, choose **Get Glooko Uploader**.
7. Use the uploader (Mac or PC) to install Glooko on your computer.
8. Once the application has been installed, connect your Omnipod Personal Diabetes Manager (PDM) to your computer using the charging cable.  
      
    **Note:** If you are using the original Omnipod (versus the Omnipod Dash), you can request a free upload cable. To make this request, call Omnipod’s manufacturer, Insulet, at [800.591.3455](tel:800.591.3455) and select Option 2.
9. When your data has been uploaded, choose **View Data**. Your data will automatically be shared with our clinic.

##### Share Your Data Using Your Mobile Device

Newer Omnipod models, like the Omnipod Dash, support sharing data using a smartphone. To share your data using your mobile device:

1. Download the Glooko app onto your device:

2. Once installed, open the Glooko app and register.
3. Choose **More**.
4. Choose **Profile** to change your settings.
5. Choose **ProConnect** and then **+ADD CODE**. (A message will pop up regarding data sharing. Choose **Continue**.)
6. Enter the ProConnect code for our clinic (**bumc**). Then choose **Submit**.
7. A message (*ProConnect Success!)* will appear confirming that you are now able to share your data with BMC. Tap **OK**.
8. To actually send your data: From the **Home** menu, choose **Sync**, then **New Device**, then **Set Up**. Choose either:
    - **Omnipod Dash System:** This option will prompt you to upload your Dash Personal Diabetes Manager (PDM) to a computer. Follow the directions to upload your data to your computer.
    - **Omnipod DASH Cloud:** This option will prompt you to log in to your Omnipod PodderCentral account. Follow the onscreen directions to upload your data.
