Consultation requests may be made through Logician, the BMC electronic chart.  **where to put this**

1. Information about you, the referring physician: name, address, office, call back telephone number if we have further questions.

2. Information about the patient, including demographics, contact information, insurance.

3. Diagnosis information: pathological diagnosis, date of diagnosis, type of diagnostic procedure.

4. Existence of any relevant records/films at outside hospitals (non-BMC) and their location. Will the patient bring any films?

5. Is the patient currently undergoing cancer treatment? What type?

6. How quickly does the patient need to be seen? Is there urgency to the request?
