I have a form that the doctor needs to fill out. How do I ensure that this form gets completed and mailed/faxed?
If you have any forms that need to be filled out by the physician, such as insurance forms or
work capacity forms, please bring these to your appointment. If you cannot wait until your
next visit, you may mail, fax, or hand-deliver the forms to our office. Please fill out our
Request for Paperwork form and attach to the front of your form. Please allow 72 hours for
forms to be completed.
