Preparing for appointment
  • Please plan to arrive 20 minutes prior to your appointment time to allow ample time to find parking and complete the registration process
  • Please bring your insurance card with you.
  • Please bring any medications, supplements and vitamins that you are taking with you.
  • Please stop on the 1st floor to register for your appointment unless you speak with a representative on the phone and are pre-registered.
  • If you need any prescription refills, please discuss this with your doctor before leaving our clinic.
  • Please be prepared to pay your co-pay at the time of your visit.
  • We appreciate 48 hours notice should you need to cancel an appointment.
  • Cell phones are allowed in our waiting room, but are prohibited in the clinic area.
