Please arrive at least 30 minutes before your scheduled appointment so that we can properly register you.

The following issues are important in preparation for your appointment.

• Bring your current medications with you or a list of your medications.
  
• Bring your insurance card.
  
• Bring copies of any medical records from previous physicians/hospitals that you have or fax them to 617-414-6800 prior to your visit.
