Services

Nursing Home Program

Our geriatrics physicians and nurse practitioners deliver primary care to elders at several of the best known skilled nursing facilities located in Boston and Quincy and are specially trained to provide care in this setting. We provide both short-term rehabilitation care as well as long-term care and work hard to ensure continuity of care and clear communication between our clinicians in the primary care and hospital settings.

Patients who are eligible to receive these services include:

• Those 65 years of age or older requiring a short-term stay for rehabilitation after hospitalization with the expectation of returning home.
  
  
• Those 65 years of age or older  requiring long term care and are nursing home residents.
