Elder's Living at Home


ERAPP: How The Program Works

  • Referrals are made via telephone or fax, and initial screening is done by Program Manager and/or Clinical Coordinator.

  • Comprehensive assessment includes health, mental health, substance abuse, functional status, financial/benefits and housing history.

  • New clients are assigned a case manager within 24 hours of entering the program, and the client and case manger design a service plan.

  • Referrals are made as needed for medical care and other services as appropriate.

  • Clients and case managers meet weekly to monitor progress and update service plan.

  • Clients are assisted with transition to appropriate next step and referrals are made for appropriate follow-up services.
