Elder Residential Assessment and Placement Program

Who the program is for:
ERAPP provides emergency housing and intensive assessment and case management services to men and woman, age 55+, who are experiencing a housing crisis or who have recently become homeless.

What the program is about:
ERAPP is a comprehensive program with the primary focus of assessing client needs and helping clients progress to the next step placement most appropriate for their needs. This can include options such as independent, congregate or assisted living. Located in the Pine Street Inn, 444 Harrison Ave., Boston, MA, clients stay in a section of the emergency shelter reserved for ERAPP clients. In addition to housing, the program assists clients in related areas, such as medical, legal and financial issues, all of which may impact their ability to attain, and maintain, permanent housing.



How the program works:
The program is designed to provide comprehensive assessment services in a safe residential setting to clients who have experienced a housing crisis. During a four to six month stay in the ERAPP, clients are expected to meet regularly with their assigned case coordinator and nurse, who will help them to identify any potential barriers to maintaining a stable living situation, and work with them to address those barriers. When appropriate, clients may also work with a housing advocate who will help them with the process of applying for subsidized housing. Each client is expected to meet the requirements of an individualized service plan, as well as follow the rules of the program and Pine Street Inn.

The process:
An interested individual should: (1) Contact

Kip Langello of the Elders Living at Home Program, (617) 414-1642, for a referral to ERAPP and (2) attend an introductory interview with ERAPP staff, which includes a tour of program space, to begin assessment of the individual's needs and abilities. If the temporary housing program is determined to be an appropriate placement for the client, staff will follow-up with the client and other service providers
