ApartmentsFast-Tracked Housing OpportunityELAHP's Housing Program is designed to assist homeless and at-risk older men and women who are interested in pursuing housing. It is a comprehensive program with the primary focus of placing clients in safe, stable housing most suitable to their needs. The program uses studio and 1-bedroom apartments in three Boston Housing Authority elderly/disabled buildings located in Dorchester, Jamaica Plain and the South End. Clients will work with a case manager and nurse who assess and monitor their ability to live independently, develop a plan for supportive services, and assist in areas which may impact their ability maintain permanent housing. Requirements • 62 years of age or older The Process Interested individuals should contact Kip Langello of the Elders Living at Home Program at 617. 414.1642. They will be asked to fill out an ELAHP referral form and a BHA application. You can download the referral form from our website. After we receive the required forms, potential clients will be scheduled for an initial interview with ELAHP staff to begin the assessment of the individual's needs and abilities.
