Nurse Case Management
The RN Care Manager is a registered nurse who coordinates care during and after the hospital stay. Acting as the liaison, he/she is the primary point of communication for the patient’s care team, as well as a resource for the patient, monitoring their complex medical needs.
Referrals are made by the RN Care Manager to other care team members including physicians, social workers, interpreters, patient advocates and many others. She/he is responsible for communicating and coordinating services beyond the acute care setting and working with the patient and family to provide a safe discharge plan. This may include home care services, procurement of health care equipment, skilled nursing and rehabilitation facilities, therapies and outpatient clinics as well as others.
How We Work With Social Workers
Case Management works closely with the Social Work team by assisting patients and their families in coping with an array of psychological and social concerns that arise throughout their care at BMC.
The role of the acute care social worker is to support the patient and family in coping with the stress accompanying the illness or injury of one of its members and enhance the adaptive capacities of the patient and family to solve its problems and make decision appropriate to the needs of the patient and family unit.
